Create order item (happy path)

Administrator requested to merge happy-order-item-creation into master

Mapping needs to be reviewed, overall much work needs to be done with the tariff parameter.

We are waiting to know how to get an account id from an existing account, so this MR still assumes that a new account is always created before applying the new order_item to it.
